Payment authorization, not charge. I understand my payment is authorized but not captured until Dr. Rachel approves my protocol. If declined, the authorization is released within 7 days and I am not charged.

Telemedicine consent. I consent to telemedicine treatment as outlined in the Terms of Service. I understand I will not have an in-person clinic visit.
